Over sixteen years ago, Walker MD Medicine and Associates D.B.A. Houma Health Clinic began its clinical practice in Houma, Louisiana specializing in Internal Medicine. Serving the Tri-Parish area with innovative patient care, our medical team strives on introducing new and trending treatment to diagnosis diseases. By being equipped with cutting edge technology and skilled medical professionals, it has kept longevity in the business.
About Us
Under the direction of Dr. Patrick D. Walker, Kristy C. Hebert FNP-C, and Eleisa N. Floyd, FNP-C, this medical staff of professionals provide overall medical treatment in the clinic, skill cared facilities, and in the hospital setting. This unique team provides adult medical care for various medical conditions: pulmonary, cardiovascular, respiratory, behavioral/psychological, or musculoskeletal.
Patient Privacy Rights
Walker MD Medicine and Associates patient’s information is protected under the Health Insurance Probability and Accountability Act of 1996 (HIPAA). This law sets rules for health providers and health insurance companies about who can look and receive patient’s information.
Patient Release of Information
Patient’s medical or health information can be released only if a written request has been done. If a patient would like a copy, place the request in writing and pay for the copying and or mailing. This request will take within 10-15 days.
